    Please find attached this year's version of the form for Parent/Guardian Input for Consideration Regarding Student Placement for the coming year.  If you wish to fill out one of these forms on behalf of your child, please return it to the office by Friday, June 8th.

    A reminder that this form is simply a mechanism for providing input to staff as one factor of many that are considered in the creation of classes for the coming year.

    Where: Bayside Middle School: Multi-Purpose Room
    What: Refreshments and goodies at 6:30; Information and Petting Zoo from 7-8 pm
